Autumn Woods Health Care Facility

I have reason to believe my mom is being neglected here. When I brought it up to the administrative staff, I got screamed at and then was told that if I don't like it, I can remove my mom and go elsewhere. Upon talking to somebody else, they said, it's gonna be investigated, and I will be told the investigation outcome. Luckily, my mom is fully with it, But I can't imagine what's happening to other patients here, that don't have people to advocate for them. I think it is completely unacceptable and unprofessional that the administrative staff of the entire building is screaming at me, I also have witnessed to this. Do NOT bring your family here PLEASE. On top of everything else, staff is not properly taken care of my mom like they're supposed to. Update: there was a nurse here that admitted to giving my mom pills that were not hers, and I think it's pretty funny that every time you go, you have to check in at the front desk So they know you're there ect ect and I used to get a bunch of how is your visit surveys, but ever since I complained, they won't let me fill any out. Thats cute. Not to mention after I did complain on their survey i got 4 phone calls that SAME DAY advising me to "remove my mom if im going to complain about there services", now those nurses found out my mom "told on them" so now they go in there with an attitude telling her that "shes hiding her pills and not taking them trying to get them into trouble" she sits for 3-6+ hours COVERED in 💩 has now been served RAW chicken. Her foley gets so full it LEAKS onto the ground and onto her! Ive been emailing/calling the facility this past week i can't seem to get a call back on email and 9/10 times i have to call 4+ times to even get to a higher ups voice mail bevause I GET HUNG UP ON. The higher up, stop checking in on my mom! I'm being ignored because I asked for a report of the nurse giving my mom medication that wasn't hers, which, by the way, still works there, along with all the other nurses that were doing this as well because it wasn't just one. Shes there for REHAB not long term care- but yet hasnt been out of bed in WEEKS. So I guess ill keep updating my review on here and even resort to posting pictures if it comes to it. I mean, for crying out loud, there's been poop on the side of my mom's bed now for OVER two months TWO MONTHS. That I have time stamped pictures on that nobody will go in there and clean! This is UNACCEPTABLE and at this point ill be calling people who will call me back. OH also is refusing to change my mom's foley done near 2+ months later! There's more but ill stop here for now. Will update again soon. DO NOT BRING YOUR FAMILY HERE IF YOU CARE ABOUT THEM! PLEASE FIND BETTER CARE! OH and vitals are almost never done. My mom's gotten no blood work sense shes been here ( what takes potassium pills and other pills that need to be monitored) and the "vitals" they put im aren't even my mom's! Saying she has "high bp" but has NEVER had a BP above 120! I was there one day with concerns because her BP was 70s/?? But yet it was put into the system almost 150/?? Update: had my mom removed- they refused to let me get my mom evaluated at the hospital, So I had to sign her out ama (or so they think they wouldnt even bother to spell her name right so its now invalid and yes I have a copy :) so ty for that) but anyways there was a lot more neglect, and I had thought once she got to the hospital.The hospital did investigate it and document everything. It was absolutely horrific what they found. As soon as I emailed the director and doctor and started talking about talking to the social worker that monday, because I was talking to them on that friday, they immediately stopped emailing me back and they then tried to do everything that they had it like, changing my mom's fully that sat there for over three months and doing blood work.And all this stuff then they started doing her vitals, consistently, like they were supposed to, and what do you know, her blood pressure was not very high. My Niece is a client in the facility and everyone in the family is always pleased with the professionalism of the staff and the cleanliness of the residents and the facility! That you for the excellent service this rendered in its entirety..it's brings great joy and peace to know that of precious family member is receiving the best care possible!

If I could give it 0 stars I would. My mom was at the skilled nursing for 3 weeks she ended up sepsis and infection in the blood and in the wound horrible place, dirty is an understatement. I truly feel so sad for other patients. DO NOT LEAVE YOUR LOVED ONES THERE. By the time we transferred my mom to the hospital because we told the nurse either you call the ambulance or we will then they decided to call and have her transferred, the ER doctor told me that if we had not brought my mom to the hospital she would have died with in hours. That’s how incompetent the staff there is. My mom has 3 months of antibiotics and still currently at the hospital. So horrible

My brother came to Autum Woods Health Care Facility for ten days. Ericka in admitting was an absolute angel, loved her! Charles was one of my brother’s care givers while there and he is also an angel!! LOVED HIM! I truly hope he does become a nurse, he would be an awesome nurse! There were several other there that were great, the women from activities were really nice and very caring. Everyone was very nice and always smiling, with that being said, I truly believe the people are extremely overworked. I would see call light on and no one would go into the room, I had pushed my brother’s once to get some help and 20 minutes later, I had to go find someone. What if I wasn’t there? There is a common area with a television in it where patients can watch TV and visit each other. Almost every time I went to visit my brother, there were quite a few employees sitting in the common area on their phones and chatting with everyone. I realize everyone needs a break, but not all at once. And I would visit with my brother for over an hour, and some of these people would still be there when I would leave. Is this why no call buttons were being answered? I tried to call the nurses station to let my brother know I wasn’t going to be there one day, and no one ever answered. I had seen that there would be people there, but they would ignore the phone. Why? What if there had been an emergency? Two nights before my brother came home, I went to see him and there were no sheets on his bed or his roommates bed. When I asked where they seets were I was told “The day shift was supposed to make the beds” that was it! I offered to help make both of the beds, but my brother was put in bed on a mattress pad instead of a sheet, Thankfully we brought him a nice warm blanket so he had something. How many other patients didn’t have sheets that night? The facility does have activities there, and the woman who came from hospice to check on him said he seemed to be enjoying the entertainment that night. There was also a superbowl pizza party that the residents seemed to enjoy. My brother and I were offere pizza and faygo sode which was very nice. Please hire more people and do not continue to work the people you have to death, it isn’t fair for anyone. Reading your other reviews, nothing I am saying here is new. Why hasn’t anything been done?

I’m so sorry for all the ones that had loved ones here. As I sit here and read all these negative comments I am shocked and my worst nightmares came true. My daughter has been in the hospital 45 days now and they came and talked to me about placing her in a nursing home after her but I said no way I will send her to any of those places I never ever heard good things about any nursing homes and told them I will be taking her home on a ventilator that’s fine we will do all the trainings and I will take care of her at home I will not put her in the hands of people that don’t care. They came to me again today talking about the training and brought up nursing home again I once again said no I will never send her to one. They said no that’s fine we r just giving you that option just in case and asked me if they could just bring a list of locations just to look. I said ok thats fine but again I wont let that happen. Anyways I was looking at the list and just curious about reviews on these places wow I was not surprised at all the negative comments I was reading I dont care to much for the positive comments cause I know its all crap lies. Thank God a million times she’s coming home with me and in my care at home safe and sound and what ever happened God For Bid atleast I know my family and I were there at all times with her taking care of her and not in the hands of these monsters that don’t care about the patients and only their for the money shame on you and shame on all these facilities that don’t care about your loved ones. How can they sit there and be so positive at the beginning and then when you admit your family there they don’t care shame on them. What about if it was one of their family members how would they like it? I’m pretty sure they won’t be happy with it. they should treat patients as a family member. I don’t see how the state is that number on this type of facility unprofessional not caring, dirty disgusting nasty bugs facility that’s falling apart workers that don’t care. I’m surprised none of you had reported to the state or if you did, I’m glad you did. Once again, I will not be bringing mine out to any of these facilities whatsoever Like I read in one of the posts, her daughter was there wheelchair-bound will get my daughter is also wheelchair-bound nonverbal. She’s on a ventilator. She will not be able to fund herself or speak up for herself. Sorry to say, but this place needs to be shut down.
