My Mother was under the care of Transitions Hospice Care beginning in December 2025 at the nursing facility where she lived. She sadly passed away last week. I cannot say enough positive things about the support both she and I received from Transitions. As my Mother’s only close person, their care was truly a lifeline for me. Having regular visits, updates, guidance, and simply an extra set of expert eyes brought enormous reassurance during such a difficult time. A very sincere thank you to Rahma Azzan, her wonderful hospice nurse, and Lauren, the social worker on the team, whose kindness and support meant so much to me. I always knew I could call Rahma, Lauren, or Transitions at any time if I had questions or needed help. I am deeply grateful for the compassionate care they provided to my Mother and to me. If your loved one needs hospice care, I would not hesitate to recommend Transitions.

It seems my review of Transitions has been removed. Please, please do more research in advance before choosing a hospice. You have alternatives. I was not at all pleased with what my mother went through at Transitions. We had a kind referral doctor. I regret not doing more research. There are certainly good, kind people here. But what happened to my mom here was so terrible and nothing like what we were promised. My mom did not deserve the long, lingering death experience at Transitions. I couldn’t get anyone to believe me. She was so clearly in pain physically and psychologically… this went on for around 14 days. My father’s death was so much better. It’s been almost 2 years since she died… of course I forgive. I don’t dwell on the experience. I wouldn’t wish what she experienced for anyone. Do your research. I regret I did not do this. Much love to all nurses who are working in this environment. The leadership… oh my… just so disappointing.

Five stars is far too few to give a hospice home like this one. I know someone who works at a different hospice home. She says this home deserves a rating of 15 on a scale of one to ten. The facility, the staff, the kindness of numerous heartfelt gifts and services we’ve encountered in just the last 24 hours have helped our whole family navigate the sad journey that is the passing of a dearly loved mom. We are thankful beyond words that we are here.

Extremely thankful for the staff, volunteers, and accommodations at transitions. Each patient’s room has a little outdoor patio, you can come and go 24/7, and you can bring pets to say their goodbyes. Amanda S., who worked with us starting at wake med is incredible and helped get us here. I don’t know how I would have made it through the past couple months without them.

Thank you to the staff at Transitions for supporting my family during such a difficult time. Elisa, the social worker, went above and beyond to communicate, show her care and keep us informed. I’ve never met a social worker as attentive, dedicated and caring as her. Amanda and David, the two nurses who cared for my dad, did an excellent job tending to his needs while showing professionalism, patience and compassion. I am forever grateful for your staff, the volunteers and the work that Transitions is doing to allow people to experience care, dignity and respect before their homegoing.
