Together Hospice Care - San Diego

Our family would like to sincerely thank Together Hospice for the care and support they provided during such an important time for us. The service was truly appreciated, and the caregiver assigned to us, Julio, was outstanding. He was compassionate, attentive, and always treated our family member with kindness and respect. His professionalism and dedication gave us a lot of comfort and peace of mind. We are very grateful for all the help and support he gave to our family.

I want to thank Together Hospice for the service they provided. It was a great help for our family member, especially with the assistance of the assigned caregiver, Julio. He was a huge help and did an excellent job very kind and professional. He helped us a lot.

I honestly don’t know what our family would have done without Joseph and the entire team at Together Hospice. We were put in an incredibly stressful position where my aunt needed to be discharged from the hospital that same day, and we were scrambling. Within just hours of our call, Joseph and his team managed to find a facility for her. Their speed and responsiveness during those first few hours took a massive weight off our shoulders and really showed how much they care about the people they work with. My aunt was no longer awake during her final week, but the level of dignity and care the nurses and caretakers showed her was amazing. Outside of her care, they navigated some very complex family dynamics with so much grace and patience, which made her stay feel that much more comfortable. They took one of the most difficult, painful times in our lives and made it as easy as it could possibly be. From the Joseph, Leslie, and everyone on the administrative side, the chaplain team, and to the frontline staff like Brenda, June, and Val, every single person we encountered was so friendly and caring. While we obviously hope we don’t have to see them again anytime soon, our family will forever remember and thank this organization for the time, care, and genuine compassion they provided. I want to sincerely thank Joseph and the entire team at Together Hospice Care for bringing my mother comfort during her last days. At first, I was overwhelmed by how hospice worked, but from the moment my mother was admitted, I felt a sense of peace knowing she was in caring hands. RN Julienne went above and beyond during her visits, ensuring my mother was comfortable and cared for, especially on her final day in which I could never give enough thanks for. Her compassion and thoroughness meant the world to me. Even though I was across the country, communication with the her was seamless, and that is something I will forever be grateful for. Any time I had a question, Julienne made sure I fully understood the process and the main goal. My mother couldn’t have been assigned a better nurse. Joseph and Together Hospice will always hold a special place in my heart—not only for finding placement for my mother, but also for helping with financial assistance when I faced challenges accessing her income. Chaplain Lisa offered words of encouragement that gave me strength, and Wendy ensured my mother was comfortable her very first nights. Without the dedication of this entire team, the process could have been far more difficult. Instead, they gave my mother dignity, comfort, and allowed me the closure I needed as well. I am deeply thankful for their care and compassion.

This hospice service was instrumental in helping my mom transition into memory care. What truly sets them apart is their deep compassion—not just for the patient, but for the entire family. Hospice care is a heavy and unfamiliar journey for many, and their thoughtful guidance made all the difference. The entire team is outstanding. It’s composed of placement agents, social workers, and nurses who work together seamlessly to provide comprehensive care. They maintained prompt and clear communication with us throughout the process, which gave our family peace of mind knowing our loved one was in good hands. We are incredibly grateful for their support during such a difficult time. Highly recommended for anyone navigating hospice care with a loved one.
